WHERE Do PGCMs Provide Services?
- Hospital -- prior to discharge
If you, or a family member, have been to the hospital recently, you probably discovered that discharge begins at admission. Hospitals are no longer for recuperation, they are for immediate, or acute, crisis management. Once a person is stable, they are politely shown the door thanks to our payor sources, the insurance companies. Discharge Planners are hospital employees who assist with your plans for immediate discharge. They are usually well versed in the community resources and facilities available, but are prohibited by their role as hospital employees to make actual recommendations. Their services also end once the patient is discharged to home. They often refer families to PGCMs when they recognize the need.
A PGCM can assist a family to personally choose the right home care or facility options based on their unique and holistic needs. Mistakes can be traumatic psychologically, emotionally, and financially.

We strive to keep clients at home, in their own familiar environment, and conduct business with our clients wherever they are. Almost everyone wants to remain in their own homes and “age in place.” PGCMs can ensure the highest quality home care, and especially when that care is needed or should be increased or decreased.
- Alternative Living Facility –
Persons in Assisted Living sometimes need more assistance than offered by the facility . ALFs are not always able to provide on-call transportation to important appointments. They do not arrange medical appointment or medical testing appointment follow-up. They do not generally accompany a resident to a medical appointment to ensure that communication is given and received and important changes to the medical Plan of Care are adhered to and understood.
