FULL COMPREHENSIVE ASSESSMENT
We assess medical needs and history, obtaining copies of medical records from all involved professionals. This ensures continuity of care and enables our nurse care managers to identify gaps in the medical care plan. Depending on the urgency of the current situation, a full comprehensive assessment might be performed as quickly as within a few hours or days. However, assessment is always ongoing. Our care managers are constantly reviewing your medical and psycho-social needs and making recommendations as change occurs. Assessments are usually performed on a flat-fee basis, determined by the complexity and urgency of the situation. |
