Here is one story...sound too familiar...
Betty forgot to pay her gas bill once too often. No one from the gas company was assigned to personally contact delinquent customers, so no one knew she was just beginning to show signs of forgetfulness.
When the gas was shut off, the house became cold overnight and, even though her neighbor checked in with her every day, she had been chilled for over 12 hours before it was discovered. Her subsequent trip to the hospital left her frightened and confused. The infection added to her confused state (actually called delirium rather than dementia, if properly diagnosed in a busy hospital).
Her family was contacted but lived in another state; there was no Power of Attorney document present at the hospital; she had one at home but had never sent a copy to her daughter. Her daughter made the frantic trip to her side in time to help choose the nursing home she needed to be discharged to for rehabilitation.
Betty's only symptom was forgetfulness. Had she been properly monitored, treated, and intervention applied, she may have avoided this incident. With the current public intervention system, she may never get back home. However, with the professional assistance of a care manager, she might be able to return home.
